In-network options
I contract with The Lactation Network to be able to provide care for folks with the following insurers:
Aetna
Anthem BCBS PPO
Blue Cross/Blue Shield PPOs
First Health
Meritain
United Healthcare (HMO & Commercial Plans)
PNOA & Multiplan (3rd Party Administrators- check for small print on your insurance card)
If you have insurance with any of the companies listed above, verify your coverage by visiting The Lactation Network at this link. They will let you know what your benefits are and handle the insurance side of things. You will not receive a bill from them or from me.
I am also in-network with:
Oregon Health Plan (open card). I will bill OHP directly for your visit(s) with me. You will not receive a bill from me.
Out-of-network options
If your insurance plan is not listed above, I am considered an out-of-network provider. You can still see me, but your appointment(s) will be self-pay. You may be able to have the cost partially reimbursed if your plan includes out-of-network coverage. Contact your insurance provider and ask about out-of-network benefits for lactation support. You may be surprised!
After your appointment(s) with me, I will provide you with a superbill and work with you to get the maximum allowable reimbursement from your insurance company. Please visit the Self-pay page for more information.
